State Disability Insurance (SDI)

State Disability Insurance is an income replacement program administered by the State of California through the Employment Development Department. Employees contribute to SDI through payroll taxes. Not all County employees participate in this program; please check your MoU or Management Resolution to see if you are eligible to participate in this program.

Eligible employees can apply for SDI if their medical provider has deemed them disabled. Please contact the Employee Benefits Services Unit to obtain a claim form. The Employment Development Department will make all determinations on eligibility as well as benefits granted. For more information of how benefits are determined please contact EDD directly at 1-800-480-3287 or visit their website at www.edd.ca.gov.

SDI Buy Back Program

County employees that participate in SDI may be eligible to participate in the buy back program. Please refer to your MoU for eligibility. All employees eligible for SDI who are off work on a medical leave are required to use all sick leave hours available to them in order to complete the pay period. Once the employee receives their SDI benefit, the employee will need to pay back any overpayments they might have received.

For example, an employee is off work for an entire month and has applied for SDI. While the employee waits for EDD to make a determination on their claim they will use their Sick Leave Accruals for that month. Once the employee receives their SDI payment, the Employee Benefits Services Unit will determine if there have been any overpayments, meaning that the employee received payment from the County and from SDI for the same period of time. If there has been an overpayment, the employee will need to buy back the sick leave hours used during the period that SDI and County Sick Leave overlapped.

According to EDD, if your doctor certifies you must limit your hours of work or modify your work duties due to your pregnancy disability you may be eligible for partial or full disability benefits. The usual disability period for a NORMAL pregnancy is up to four weeks before the expected delivery date and up to six weeks after the actual delivery. However, your doctor may certify to a longer period if the delivery is by Cesarean section, if there are medical complications, or if you are unable to perform your regular or customary job duties.

Paid Family Leave (PFL)

The Paid Family Leave program provides employees partial wage replacement when taking time off work to care for parents, children, spouses and registered domestic partners or to bond with a new minor child. Employees appointed to regular and temporary appointments including faculty, staff, administrators are paid on a monthly basis usually around the first of each month. Pay for student assistants, casual workers and overtime pay is usually issued around the middle of each month. The California State Controller’s Office has provided an online Paycheck Calculator to calculate net pay and withholding amounts for employees paid on a monthly basis.

2016 Direct Mailing of Form W-2 Wage Tax Statement

  • The Form W-2 Wage Tax Statement reflects all wages issued/paid during the 2016 tax year regardless of the pay period in which the wages were earned.
  • The Form W-2 includes warrants/payments issued January 1 st through December 31 st of each calendar year. It contains all wages and tax information for an employee regardless of the number of State agencies/campuses for which he/she worked during the tax year. Only one Form W-2 is issued by the State Controller’s Office for each “State” employee.
  • The State Controller’s Office (SCO) in Sacramento will print and mail employee’s 2016 W-2 Forms by January 22, 2017, to the employee’s address on file with the Payroll Office as of December 12, 2016.
  • Employees who do not receive their W-2’s within a couple of weeks after the SCO issue date may request that Payroll order a duplicate W-2 on their behalf (fee charged to campus). This is provided their address is correct in the Payroll system and the written request for the duplicate is received by Payroll prior to March 1, 2017. (Please use form indicated below to request a duplicate W-2)
  • Employees who do not receive their W-2’s by February 28, 2017 and did not request that Payroll order duplicate W-2’s on their behalf, will be required to request duplicate W-2 forms directly from the State Controller’s Office W-2 Unit at a cost of $8.50. The SCO estimates approximately a 2 to 3 week turn-around period during the tax period. Payroll cannot request duplicate W-2’s on behalf of employees after February 29, 2017.

Requesting a Duplicate W-2 Form:

In the event you lose or require a duplicate W-2 Form for any other reason, you may request a duplicate copy of a W-2 through the State Controller’s Office.

    The cost of this program is $502 per credit hour, with no additional program fees. Technology, Financial Aid Trust and other mandatory university fees may apply.

    Applicants must fulfill the requirements of both the Graduate College and the College of Public Service and Community Solutions.

    Applicants are eligible to apply to the program if they have earned a bachelor’s or master’s degree in criminology or criminal justice, public administration or public service, sociology or social work, public safety or fire service management, parks and recreation management, applied sciences or another closely related field from a regionally accredited college or university.

    Applicants must have a minimum of a 3.00 cumulative GPA (scale is 4.00 = “A”) in the last 60 hours of a student’s first bachelor’s degree program, or applicants must have a minimum of a 3.00 cumulative GPA (scale is 4.00 = “A”) in an applicable master’s degree program.

    All applicants are required to submit:

    • Graduate application and application fee
    • Official transcripts
    • Three letters of recommendation
    • Proof of English proficiency

    Additional Admission Information

    An applicant whose native language is not English (regardless of current residency) must provide proof of English proficiency.

    I wanted to know if anyone happens to know of an accelerated bsn program that does not require a previous bsn degree, and does not have a wait list. I know I am asking for alot, however I know there has to be one out there . I am willing to move anywhere. I have been calling state to state and am still not having much luck . I have completed Anatomy, Physiology, Microbiology, Chemistry, Nutrition, and Lifespan Development. I also scored an 80% on my TEAS exam. The schools I have come across either require a previous bsn or more pre-reqs like public speaking or Statistics. I know I might just need to bite the bullet and take those classes, however I feel there has got to be a private or public school out there that will accept me for what I have. Any help would be much appreciated. Thank you all.

    Thanks for all your insight. I do not have my RN or a Batchelors Degree. I have completed 65 credits of general ed as well as most pre-reqs required for Nursing beside Statistics and Public Speaking.     I think most of them require stats and public speaking. We’re using stats now in my research class. I’m in an LPN/BSN bridge.

    Hypnosis — or hypnotherapy — uses guided relaxation, intense concentration, and focused attention to achieve a heightened state of awareness that is sometimes called a trance. The person’s attention is so focused while in this state that anything going on around the person is temporarily blocked out or ignored. In this naturally occurring state, a person may focus his or her attention — with the help of a trained therapist — on specific thoughts or tasks.

    How Does Hypnosis Work?

    Hypnosis is usually considered an aid to psychotherapy (counseling or therapy), because the hypnotic state allows people to explore painful thoughts, feelings, and memories they might have hidden from their conscious minds. In addition, hypnosis enables people to perceive some things differently, such as blocking an awareness of pain.

    Hypnosis can be used in two ways, as suggestion therapy or for patient analysis.

    • Suggestion therapy: The hypnotic state makes the person better able to respond to suggestions. Therefore, hypnotherapy can help some people change certain behaviors, such as stopping smoking or nail biting. It can also help people change perceptions and sensations, and is particularly useful in treating pain .
    • Analysis: This approach uses the relaxed state to explore a possible psychological root cause of a disorder or symptom, such as a traumatic past event that a person has hidden in his or her unconscious memory. Once the trauma is revealed, it can be addressed in psychotherapy .

    What Are the Benefits of Hypnosis?

    The hypnotic state allows a person to be more open to discussion and suggestion. It can improve the success of other treatments for many conditions, including:

    Hypnosis also might be used to help with pain control and to overcome habits, such as smoking or overeating. It also might be helpful for people whose symptoms are severe or who need crisis management.

    What Are the Drawbacks of Hypnosis?

    Hypnosis might not be appropriate for a person who has psychotic symptoms, such as hallucinations and delusions. or for someone who is using drugs or alcohol. It should be used for pain control only after a doctor has evaluated the person for any physical disorder that might require medical or surgical treatment. Hypnosis also may be a less effective form of therapy than other more traditional treatments, such as medication. for psychiatric disorders.

    Some therapists use hypnosis to recover possibly repressed memories they believe are linked to the person’s mental disorder. However, the quality and reliability of information recalled by the patient under hypnosis is not always reliable. Additionally, hypnosis can pose a risk of creating false memories — usually as a result of unintended suggestions or the asking of leading questions by the therapist. For these reasons, hypnosis is no longer considered a common or mainstream part of most forms of psychotherapy. Also, the use of hypnosis for certain mental disorders in which patients may be highly susceptible to suggestion, such as dissociative disorders, remains especially controversial.

    Is Hypnosis Dangerous?

    Hypnosis is not a dangerous procedure. It is not mind control or brainwashing. A therapist cannot make a person do something embarrassing or that the person doesn’t want to do. The greatest risk, as discussed above, is that false memories can potentially be created and that it may be less effective than pursuing other, more established and traditional psychiatric treatments.

    Who Performs Hypnosis?

    Hypnosis is performed by a licensed or certified mental health professional who is specially trained in this technique.

    Exemption of Continuing Competency Requirements

    The Virginia Board of Physical Therapy in accordance with Section 18VAC112-20-131(C) of the Regulations, entitled Continued Competency Requirements for Renewal of an Active License , exempts a licensee from the continuing competency requirements for the first biennial renewal following the date of initial licensure by examination in Virginia. This exemption does not include those who were licensed by endorsement.

    Extension of renewal requirements for deployed military and spouses
    Virginia law allows active duty service people or their spouses who are deployed outside the U.S. to have an extension of time for any requirement or fee pertaining to renewal until 60 days after the person s return from deployment. The extension cannot last beyond 5 years past the expiration date for the license. For more information, please read attached policy and contact the applicable board for your license.

    Ohio State University joins massive open online course movement

    COLUMBUS, Ohio – Ohio State University. in the past reluctant to offer online courses, is now part of the massive open online course movement known as MOOC.

    OSU announced last month it would join Coursera, which offers free non-credit online classes. Coursera added 17 new university partners for a total of 33.

    Some of the top institutions in the country, including Princeton University, Stanford University, Duke University and Johns Hopkins University, will offer classes through Coursera.

    “You’re known by your partners, and this is the College of Cardinals,” OSU President E. Gordon Gee told the New York Times. “It’s some of the best universities in the country.”

    Gee said in the interview he had some concerns about providing courses at no cost.

    “That does keep me up at night,” he said. “We’re doing this in the hope and expectation that we’ll be able to build a financial model, but I don’t know what it is. But we can’t be too far behind in an area that’s growing and changing as fast as this one.”

    The Chronicle of Higher Education has called 2012 the “year of the mega-class” as companies, some for-profit, have been founded by elite institutions.

    Interest in MOOCs became keen after the Massachusetts Institute of Technology launched MITx in December, 2011. More than 150,000 students worldwide enrolled in its prototype course on circuits and electronics this spring.

    Coursera was founded by two Stanford University professors in April. It has registered 1.57 million students and currently offers 198 courses.

    In May MIT partnered with Harvard University to form edX. The University of California at Berkeley joined in July and the company offered seven courses this fall.

    Other companies are Udacity. founded by a Stanford University computer science professor; Udemy, which allows anyone to set up an online course and Khan Academy, which offers more than 3,400 videos geared toward elementary and secondary students.

    The MOOC movement is so new that educators are unsure how it will affect traditional higher education. The online classes generally have little faculty involvement. Students watch lectures and complete assignments that are often graded by machines or students.

    Although universities can offer online courses to enrolled students for credit, the ability to reach tens of thousands of students through MOOC is attractive to professors and provides visibility to the institution. And it may one day provide some income.

    Officials say the free courses provide access to everyone and improves learning. However, only 7,000 of the 150,000 who enrolled in MIT’s course completed it an earned a certificate.

    Gee often has said he prefers students take classes for credit on a university campus rather than online and mocks a commercial in which a women in pajamas says she’s getting a college degree while at home.

    He said a college experience cannot be replicated online.

    “Online is ridiculous,” he said in a 2010 interview. “Places like Ohio State bring people here together and students teach each other. They are in class 18 hours and on their own for 150 hours (each week). That cannot be replicated online.”

    OSU currently offers a handful of graduate programs online and a few courses for undergraduates, said Wayne Carlson, vice provost for undergraduate studies.

    “Online is an approach to teaching and learning that we can’t ignore,” he said. “We have to pay attention to it and are looking at what it means to us as an institution and how to embrace it and maintain the quality of the education process that we believe in.”

    He said OSU officials watched the development of MOOCs and saw its potential to change the way courses are delivered.

    “We realized in order to be at the table to discuss how this approach to higher education takes place we really needed to participate rather than wait on the sidelines and watch others do it,” he said.

    He said Gee asked him to investigate Coursera after he met one of the founders.

    OSU will offer two courses next year on Coursera. Those who complete them will receive a certificate signed by the online instructor

    One is a 10-week long introduction to pharmacy, an existing course that will be adapted to an online format. The other is a six-week course on the science behind prescription drug abuse, which has been taught as an outreach program in high schools.

    “Some faculty believe that everyone in the world deserves to participate in higher education and it shouldn’t be limited to those who can afford it or have the ability to go to campus,” Carlson said of the benefits of Coursera. “Others say it is an opportunity to get our message out there.”

    He said some officials believe joining the reputable universities in Coursera gives OSU “street cred.”

    “There is some truth to all of those things,” he said.
