The first thing you should do after reading someone’s answer to your question is vote on the answer, like any other user (with sufficient reputation) does. How do I write a good answer?

Pay it forward

Have the same problem?

Still no answer to the question, and you have the same problem? Help us find a solution by researching the problem, then contribute the results of your research and anything additional you’ve tried as a partial answer. That way, even if we can’t figure it out, the next person has more to go on. You can also vote up the question or set a bounty on it so the question gets more attention.

Answer the question

Read the question carefully. What, specifically, is the question asking for? Make sure your answer provides that – or a viable alternative. The answer can be “don’t do that”, but it should also include “try this instead”. Any answer that gets the asker going in the right direction is helpful, but do try to mention any limitations, assumptions or simplifications in your answer. Brevity is acceptable, but fuller explanations are better.

Provide context for links

Links to external resources are encouraged, but please add context around the link so your fellow users will have some idea what it is and why it’s there. Always quote the most relevant part of an important link, in case the target site is unreachable or goes permanently offline.

Write to the best of your ability

We don’t expect every answer to be perfect, but answers with correct spelling, punctuation, and grammar are easier to read. They also tend to get upvoted more frequently. Remember, you can always go back at any time and edit your answer to improve it.

Not all questions can or should be answered here. Save yourself some frustration and avoid trying to answer questions which.

    Hypoplastic left heart syndrome

    Hypoplastic left heart syndrome (HLHS) refers to the grouping of certain congenital cardiac defects. The main characteristics are the marked hypoplasia (underdevelopment) or even absence of the left ventricle and severe hypoplasia of the aorta. Often a localized pinching (coarctation) of the aorta is also present. The main pulmonary artery is enlarged, and gives rise to a large ductus arteriosus. This allows blood to flow from the right ventricle into the aorta and out to the body. Other characteristics of HLHS often include a combination of aortic and mitral stenosis or aortic and mitral atresia.

    The lack of a developed left ventricle, plus the aortic coarctation leads to reversed blood flow through the aorta. Partially oxygenated blood reaches the aorta after travelling through the patent foramen ovale, up the pulmonary trunk and through the patent ductus arteriosus. The major blood flow to the systemic circulation is through the PDA.

    HLHS is also used to describe cases in which the right ventricle is hypoplastic and the child also suffers from transposition of the great arteries. In such cases the hypoplastic right heart pumps into the same artery that the left ventricle normally pumps blood through, and therefore the circulation is the same as in HLHS. This is sometimes referred to as functional HLHS.

    De-oxygenated blood flows from the right atrium through an atrial septal defect into the left atrium. There it mixes with oxygenated blood returning from the lungs. A portion of this blood flows from the left ventricle up the pulmonary trunk to the lungs and through the patent ductus arteriosus to the body. Another portion of this blood flows through a ventricular septal defect, and is pumped by the hypoplastic right ventricle up the aorta.

    HLHS is a severe form of congenital heart disease. Without surgical intervention, HLHS is fatal. Infants are often diagnosed within 24 to 48 hours of birth. Symptoms appear when the ductus arteriosus begins to close.

    There are three options for treating these children: supportive care until death occurs, staged reconstruction of the heart, or a heart transplant.

    Before surgical treatment was developed, at least 90 per cent of infants with this condition died by the age of one month. Even today, some infants are not candidates for surgical therapy. For these infants, supportive care is the only option. Exactly what this care entails will depend on the condition of the infant, but would likely include assistance with breathing, the provision of fluids and management of pain.

    Reconstruction of the heart

    Staged reconstruction is the treatment of choice for hypoplastic left heart syndrome. Reconstruction takes place in three stages.

    The first stage, known as a Norwood operation, is undertaken as soon as possible after birth. This stage calls for a homograft 1 to be used to enlarge the rudimentary aortic arch and then join it to the pulmonary trunk. This is known as an anastomosis. The pulmonary trunk will have been disconnected from the left and right pulmonary arteries. The newly created blood vessel functions as the patient’s aorta. The right ventricle then becomes a common ventricle, pumping blood through the aorta to the rest of the body.

    Blood flow to the lungs is provided by means of a modified Blalock-Taussig shunt joining the innominate artery to the pulmonary artery. This increases the flow of oxygen-enriched blood that will be sent to the body. The Norwood operation also involves making a hole in the wall between the left and right atria. In medical terms this is known as an atrial septectomy.

    Blood flows through the anastomosed aorta and pulmonary trunk to the aortic arch. A shunt connects the aorta to the pulmonary arteries, providing the lungs with blood to oxygenate.

    After the Norwood operation, children require medical management with various drugs. Some drugs, such as digoxin, are needed to improve the strength of the heart’s contractions, and other drugs, such as diuretics, are required to remove excess fluid from the body.

    The second stage of reconstruction is undertaken between three and ten months of age. Stage two involves performing a bidirectional Glenn shunt or a hemi-Fontan. (This is also known as a partial Fontan or a Bidirectional Cavo-Pulmonary Shunt.)

    A bidirectional Glenn shunt is performed by connecting the superior vena cava (SVC) to the right branch of the pulmonary artery. As a result, venous blood from the head and upper limbs will pass directly to the lungs, bypassing the right ventricle. However, the venous blood from the lower half of the body will continue to return to the heart.

    While avoiding the risk of failure of a complete Fontan operation, the partial Fontan helps to relieve symptoms. The procedure decreases the volume of blood delivered to the single ventricle, thus reducing the amount of work that the ventricle must perform. Post-operatively, oxygen saturation is improved.

    In addition, because the bi-directional Glenn is a low-pressure shunt, it does not carry the risk of causing thickening and hardening of the blood vessels of the lungs. (This is a normal response of the lung’s blood vessels to high pressures.) This operation creates a more favorable setting in which to complete a Fontan reconstruction at one and a half to two years of age.

    De-oxygenated blood from the superior vena cava is routed directly to the lungs. A mix of oxygenated and de-oxygenated blood is sent to the body through the anastomosed aorta and pulmonary trunk.

    At 18 to 24 months of age, if the patient does well after the Glenn shunt and pulmonary resistance remains low, the third stage of reconstruction is possible. This involves the completion of the Fontan procedure (as described in the earlier discussion of tricuspid atresia).

    With the Fontan operation, the venous blood from the lower part of the body is also diverted to the lungs, thus creating the ‘complete’ Fontan circulation. However, if the child is not well or the pulmonary resistance is considered too high, then no further surgical treatment is possible.

    De-oxygenated blood from the superior vena cava and inferior vena cava is routed directly to the lungs. Oxygenated blood is sent to the body through the anastomosed aorta and pulmonary trunk.

    Heart transplantation is carried out using the same basic techniques as for other transplants. In the case of pediatric heart transplants, the greatest problem is that infant donor hearts are in short supply. Thus transplantation is a treatment for only a limited number of babies with hypoplastic left heart syndrome. In addition, recipients can survive only with the assistance of heavy doses of expensive medication to prevent rejection of the transplanted tissue. These drugs also have significant side-effects, such as the risk of infection and lymphatic cancer, and must be taken by recipients for the rest of their lives. For those reasons, heart transplants are not considered the standard treatment for children suffering from HLHS.

    A homograft is a graft of tissue between animals of the same species. In the case of humans, a homograft comes from a tissue donor. A homograft is in contrast to an autograft, which is a graft of tissue transferred from another part of the patient’s body. With pulmonary homografts, the pulmonary artery is removed from a tissue donor shortly after death. The pulmonary homograft is prepared and stored indefinitely at -70 degrees Celsius in liquid nitrogen until needed, when it is thawed and prepared under sterile conditions .

    Hyperemesis gravidarum is a severe form of morning sickness that is characterized by excessive nausea and vomiting, leading to dehydration and dizziness. It affects 0.5% to 2% of pregnant women.

    Dizziness is an umbrella term that is used to signify lightheadedness, disorientation, or imbalance. Nausea is the sensation of uneasiness that is accompanied by a strong urge to vomit. Nausea and dizzy spells are often experienced by people who are susceptible to motion sickness. Women may also experience these symptoms during the course of pregnancy.

    However, it must be noted that these could be the indicators of certain medical conditions that require immediate attention. Symptoms that may accompany lightheadedness and nausea should therefore be considered for an accurate diagnosis.

    Hyperemesis gravidarum
    Most women experience dizziness and nausea during pregnancy, especially during the first trimester. This condition is known as morning sickness, and is considered to be absolutely normal. However, hyperemesis gravidarum is a severe but rare form of this morning sickness. It is characterized by severe vomiting and nausea. The excessive vomiting may result in weight loss, dehydration, nutritional deficiencies, electrolyte imbalance, and dizzy spells.

    Vertigo
    The term ‘vertigo’ refers to a spinning sensation. The affected individual feels that he/she or the surroundings around him/her are in motion. Such a spinning sensation causes disorientation. The affected individual may also feel nauseated. Abnormal eye movements, nausea, vomiting, or sweating are some of the symptoms that may accompany disorientation. Benign Paroxysmal Positional Vertigo (BPPV) is a vestibular disorder that is characterized by a sense of imbalance, nausea, and lightheadedness. There is a sudden onset of the symptoms with a change in the position of the head.

    Motion Sickness
    Motion sickness, which is commonly referred to as travel sickness, is a nauseating sensation that some people may experience while traveling by car, train, ship, or an airplane. It is caused by the disturbance or conflict between the signals that are sent to the brain by the eyes, vestibular system (inner ear and the cochlea), and the sensory receptors that are located in the joints, tendons, and other body tissues. When the brain gets contradictory signals regarding the position of the person, the affected individual experiences symptoms such as dizziness, nausea, and vomiting.

    Panic Attacks
    A panic attack is characterized by sudden onset of intense fear that could give rise to symptoms such as dizziness, racing heart, sweating, nausea, chest pain, sensation of shortness of breath, etc. People may be predisposed to panic attacks in certain situations. Those who suffer from phobias, post-traumatic stress disorder, and hyperventilation syndrome are more likely to suffer from panic attacks.

    Medication
    Use of certain drugs can cause dizziness and vomiting. Drugs that are prescribed for treating hypertension may cause dizziness. Angiotensin-converting enzyme inhibitors and antidepressants could also cause dizziness. Abrupt withdrawal of certain drugs could also be a contributory factor.

    Dysmenorrhea
    Dysmenorrhea is a medical term that is used to signify painful menstruation. It can cause dizziness, nausea, vomiting, and abdominal cramps in some women. It is believed that release of prostaglandins from the uterine lining gives rise to these symptoms.

    Gastrointestinal Problems
    Nausea and vomiting are some of the most common symptoms of gastrointestinal problems such as diarrhea, food poisoning, and gastroenteritis. Severe vomiting can lead to dehydration, which may give rise to dizzy spells.

    Cardiovascular Problems
    Irregular beating of the heart muscles, arrhythmia, heart attack, cardiac arrest, sudden decrease in blood pressure, etc. alter sufficient supply of blood and oxygen to the tissues of the brain. This results in dizziness followed by an immediate loss of consciousness. In addition, the blockage of arteries that carry blood to the brain (carotid arteries and its branches) may also lead to dizziness. Consistently high blood pressure levels may also give rise to headaches, dizziness, nausea, and excessive sweating.

    Respiratory Disorders
    Respiratory disorders and infections in the respiratory tract may lead to difficulty in breathing, and the resultant low intake of oxygen often leads to dizziness. These include obstructive pulmonary disorders, asthma attacks, pulmonary edema, etc.

    Neurological Disorders
    Brain and spinal cord injury, as well as damage and inflammation of cranial nerves, are crucial etiological factors as far as dizziness is concerned. These conditions alter the conduction of signals necessary for the proper sense of balance.

    Olfactory Problems
    Inner ear is an important organ that plays a vital role in maintaining the sensation of balance and equilibrium. Infection, inflammation, fluid buildup, and injury to these tissues leads to dizziness and vertigo.

    SAT by MBA Learning Center Offers High-Quality Academic Programs for the Upcoming Spring Summer

    MONTVILLE, NJ – Spring has finally arrived and the summer is approaching! Located on New Road in Parsippany, SAT by MBA Learning Center offers a series of high-quality but low-cost academic programs to benefit local students in North Jersey areas.

    SAT by MBA offers small group classes (3-6 students) as well as one-on-one private tutoring; we have a flexible schedule and can tailor our program to fit your child s needs.

    During the school year we offer more than 15 academic courses in various areas (see listing below). Other than the academic courses, in summer the forensics coach of Montville Twp. High School (MTHS) will offer a 2-week (20 hours) intensive training camp in person at our center to help students from other communities. The coach has 27-years of experience of leading MTHS Forensics Club and helps the students win numerous awards every year.

    Starting March 30. a new series of SAT classes targeting May and June SAT tests will kick off. The classroom location for Monday/Thursday s SAT programs will be held at Montville Community Park s Youth Center at 130 Change Bridge Road, Montville, NJ 07045. Other classes will be held at SAT by MBA s office at 141 New Road, Parsippany, NJ 07054.

    The SAT program is designed by SAT by MBA Learning Center to offer students the ultimate solution to maximize SAT scores effectively and efficiently. The program s goal is to help students with their academic path in a time-conscious way. Each session will cover either SAT math or SAT verbal (including critical reading/writing). The students may choose one math only and/or one verbal only sessions to target their individual needs. For new students who have never taken any SAT tests and have never had any SAT training, we highly recommend them to register for both subjects. There will be 3 to 4 free optional proctored mock exams to be held at the SAT by MBA Learning Center in Parsippany. A Q A session will also be offered after the mock exams.

    Students will be required to secure an SAT text book The Official SAT Study Guide , 2 nd edition published by the College Board (ISBN-13: 978-0874478525). For our summer SAT prep camp, another new SAT book published by the College Board will be needed if you plan to take the Redesigned SAT which will be offered starting March 2016. The book, Official SAT Study Guide (2016 Edition, ISBN-13: 978-1457304309), will hit the market on June 30 this year.

    As we know, every student has his/her own strength and weakness. The one-to-one private lesson is the best way to focus on each individual student s needs. If the students need additional help on the side to advance to a higher level, we can help them arrange private lessons or another advanced/small group classes at the facility located in Parsippany, NJ.

    All of the instructors at SAT by MBA Learning Center are experienced SAT teachers and hold at least one master and/or one Ph.D. degree; most of them were graduated from Ivy League schools or first-tier colleges.

    Most of the teachers at SAT by MBA are also experienced in teaching SAT II/ACT/AP/GRE tests as well as all middle school and high school academic subjects.     AIG, JPMorgan expanding BPO operations here despite crisis

    THE LOCAL outsourcing operations of troubled US insurance firm American International Group (AIG) and financial giant JPMorgan Chase Co. will continue expanding here despite mounting concerns of a slump in the world�s largest economy.

    AIG-Business Processing Services, Inc. (BPSI), which has operations in the Philippines handling all customer service requirements of its parent, said it does not expect demand to slow down. The company is hiring 1,700 more workers for a third business process outsourcing (BPO) site.

    Despite the downturn, there is still a huge opportunity since AIG is still the largest insurance company in the world, AIG-BPSI Chief Executive Officer Chris Duncan-Webb told executives at the Asia CEO Forum in Makati.

    In a separate briefing, JPMorgan said it was expanding its BPO operations here to serve its American credit card clientele.

    Chase, the New York-based financial giant�s brand for its credit card services, yesterday opened its 14,050-seat call center facility, which will be housed at the New Quad Building in Fort Bonifacio in Taguig City.

    The launch marks the first phase of JPMorgan�s expansion plans in the country, five years after it tapped the Philippines for its offshore backroom operations. JPMorgan has been housed at the Philamlife Tower in Makati City since 2003.

    Last week, the US Federal Reserve extended a lifeline worth $85 billion in loans � designed to help AIG meet its obligations and prevent its collapse � in exchange for an 80% stake in the company.

    Fed officials said they had to act because of AIG�s extensive involvement in financial markets.

    Through its insurance, risk and asset management businesses, AIG has dealings with thousands of companies all over the world, so a bankruptcy would have had huge global repercussions.

    As a result of the US home loan mortgage crisis, AIG has lost more than $18 billion since the year started. Its share price has fallen by more than 90%.

    On the other hand, JPMorgan did not sustain as much damage from the US subprime mortgage mess by avoiding bad mortgage investments and focusing instead on its commercial and retail banking businesses.

    AIG�s Mr. Duncan-Webb said the company had set up its third BPO site in Muntinlupa City, which is expected to more than double the company�s employees.

    BPSI, which has 1,200 workers now, is planning to add 1,700 more employees when the site opens in December.

    He said the company had prepared the site ahead of the need, in anticipation of higher demand from its parent in the next few years.

    Mr. Duncan-Webb said the Philippines has an incredible future ahead of it, even with its small operations in the global BPO sector.

    He said the country is a good investment site for the long term. The company, he added, is confident that demand for its services from AIG would not slow down.

    He also said outsourcing services have evolved into knowledge-based services from regular call center operations.

    Mr. Duncan-Webb said AIG would find it hard to bring the outsourced services back to the US, where labor costs are higher.

    The insurance firm is a Fortune 500 company that has over a trillion dollars in assets and around 74 million clients in 130 countries.

    Meanwhile, JPMorgan said its expansion attests to its increased optimism in the Philippine economy.

    Over the years, JP Morgan will be a top employer here. This marks only the first phase of our expansion in the Philippines. The best is yet to come, Barry E. Marshall, JPMorgan senior country operations officer, said at yesterday�s ribbon-cutting ceremony.

    Our investment in this facility showed that we�ve done well, said Michael J. Looney, JP Morgan card service operations executive.

    JPMorgan plans to grow its call center manpower to 24,000 in the long term, starting off with 2,000 by yearend. It now has 1,300 workers.

    It depends on where customer demand is, Mr. Looney said in an interview. We will need more staff as we grow the business, he added.

    The credit card arm of JPMorgan has around 50 million customers in the US. While admitting that it has been a more challenging year � with more Americans defaulting in their loan payments and the widening credit squeeze in the US dampening consumer demand � Mr. Looney was bullish about growing their credit card portfolio.

    We�re not immune to [defaults]. But we�re well positioned to weather the crisis, he said.
    JP Morgan has veered away from subprime mortgage-related investments, shielding it from the financial crisis that had led to the collapse of America�s financial market pillars like Lehman Brothers.

    The Philippines wants to corner a 10% share of the projected $130-billion BPO market by 2010. Business process outsourcing was a $3.3-billion industry as of 2007, with 120 companies employing at least 200,000 workers.

    ROUTINE SPEEDING STOP GOES OUT OF CONTROL

    In the early morning hours of December 6, 2009, my whole life changed. I was the Municipal Court Judge for the City of Wentzville, member of a family of extremely well-known REALTORS, and a hard working criminal-defense attorney. I had never faced real criminal charges of any sort, and was/am a law-abiding citizen who took pride in contributing to my community.

    So, imagine my surprise when, while driving home from a late night at the office, an officer pulled me over for speeding, but didn’t stop there. At first I thought it was a routine speeding ticket and was prepared just to wait for the paperwork — I own Traffic Law Counselors® and am familiar with the process. However, the trooper started asking me questions about what I’d had to drink that evening; to which I replied, “nothing.”

    OFFICER SAYS GET OUT OF THE CAR

    Still, the officer asked me to get out of the car and did not like it when I asked, “why?” — later he admitted at trial that he didn’t like someone questioning his authority. By the way, I never (not once) mentioned I was an attorney or judge. Regardless, I pretty much knew at that moment that the officer had decided to arrest me for DWI. After many, many past depositions, officer interviews and bench trials (as judge and attorney) this much was clear to me.

    I’ve seen it a number of times, a person is singled out for something as minor as driving a little over the speed limit, and because of the hour and/or some other so-called “indicator” of intoxication, the officer has already made up his mind that the driver is drunk. In my case, the indicator was red, bloodshot eyes. Now, bloodshot eyes at 1:00 in the morning after hours of staring at a computer screen isn’t at all surprising to me, but to an officer who has seen firsthand the horrors that can result from having actual drunk drivers on the road, any possible indicator can be enough. A lot of the time these officers err on the side of caution, not thinking of the gross violation of the driver’s rights that they are imposing.

    I KNEW MY CONSTITUTIONAL RIGHTS, OFFICER DIDN’T LIKE THAT

    As an attorney, I’m well aware of my constitutional rights and the way that they work, so I chose to exercise mine at that time. I didn’t submit to any testing and I didn’t answer any questions (even though I hadn’t been drinking), because I know better than anyone how easily your words can be twisted, and how the Standardized Field Sobriety tests are set up in a way to almost ensure that you will fail. In fact, to the trooper’s credit, his report and testimony did accurately reflect that I had always denied having anything to drink that evening.

    Even though the officer had no evidence against me, other than bloodshot eyes and what he says was an odor of an intoxicating beverage, he placed me under arrest for DWI, and the rest of my life spun out of control as a result.

    MY CHARGES AND LIFE WERE BROADCAST ALL OVER THE NEWS

    Aware of one of the first rules of lawyering learned in every law school — “The Attorney Who Represents Him/Herself Has A Fool For A Client ” — I hired a good DWI attorney outside of my firm, knowing that I was going to fight the bad charges against me in a trial if the charges weren’t dropped (which I had mistakenly presumed would happen). Many people choose to take a plea bargain (which I could have done and was urged to do at one point), but I knew that I could not in good conscience plead guilty to something that I did not do.

    And, because really fighting a bad DWI charge in our courts takes a LOT of time (especially if you are exploring ALL of your legal options/arguments), I suffered the embarrassment of having my charges broadcast through all the local media outlets — NBC, StlToday, Suburban Journals, Patch, CBS, ABC, and more. There was even speculation as to why my trial was being postponed which added to the pressures of properly performing my duties as a Municipal Court Judge at the time.

    learned first hand what it’s like to have one’s personal life exposed to the world. That type of attention sure doesn’t help any in dealing with the sinking feeling you already have when thinking about all the uncertainties that seem to come with a DWI charge.

    So, now I help people KNOW their OPTIONS, worry much less, get fair outcomes and keep their driver’s license.

    MY FAMILY SUFFERED, YET I DIDN’T TAKE A PLEA BARGAIN

    My family suffered as well. My wife had to deal with questions and comments from her co-workers, and my kids didn’t know what to make of the things their dad was being accused of. One time a co-student found my online story and brought it to the attention of my kid during class at school 🙁

    In spite of all this, I didn’t cave to the plea offers that could have brought a quicker end to all of the scrutiny — which most successful public relations professionals recommend for elected officials (remember when Mizzou Coach Gary Pinkel pled guilty in less than 24-48 hours?). Instead, I chose to pursue reelection as judge in Wentzville, unashamed and unswayed by the negative attention and mudslinging of my election opponents

    A JURY FOUND ME INNOCENT IN 9 MINUTES

    Six days after losing reelection as judge because of these DWI charges, the jury in my criminal trial came back after a mere 9 minutes of deliberation, declaring me innocent of the DWI charge brought against me. Even though I was found innocent of the charge, my life was changed drastically just by the accusation.

    I like to believe that I have always approached my clients’ legal matters in the same way that I would if they were my brother or mother; but I know without a doubt that having personally faced a DWI charge, I have a whole new perspective on how serious a matter it really is. I never allow myself to forget that when I’m dealing with the charges my clients face.

Two certified nurse examiners are certified for the care of both adult and pediatric patients, providing medical and legal exams of patients who present with forensic health care needs; maintaining chain of custody with medical and legal documentation and storage; and are responsible for testifying as expert witnesses in a court of law. These registered nurses received specialized classroom and clinical training in the evaluation and treatment of sexual assault in patients who have experienced or are experiencing violence or abuse. The patient also receives resources and referrals. All mandatory healthcare professional reporting requirements are met and documented. The SANE program interfaces with medical professionals, law enforcement agencies, criminal justice, victim advocacy, and external support agencies that provide services to victims of sexual assault.

    Cooling infrastructure is a significant part of a data center. The complex connection of chillers, compressors and air handlers create the optimal computing environment, ensuring the longevity of the servers, and the vitality of the organization they support.

    Yet, the current data center cooling ecosystem has come at a price. The EPA s oft-cited 2007 report predicted that data center energy consumption, if left unchecked, would reach 100 billion kWh by 2011 with a corresponding energy bill of $7.4 billion. This conclusion, however, isn t strictly based on Moore s Law or the need for greater bandwidth. Their estimate envisions tomorrow s processing power will be addressed with yesterday s cooling strategies. The shortcomings of these designs, coupled with demand for more processing power, would require (10) new power plants to provide the juice for it all, according to that report.

    According to a more recent study commissioned by the NY Times from Jonathan Koomey Ph.D. Stanford entitled, Growth in Data center electricity use 2005 to 2010, the rapid rates of growth in data center electricity use that prevailed from 2000 to 2005 slowed significantly from 2005 to 2010, yielding total electricity use by data centers in 2010 of about 1.3% of all electricity use for the world, and 2% of all electricity use for the US. Assuming the base line figures are correct, Koomey states that instead of doubling as predicted by the EPA study, energy consumption by data centers increased by 56% worldwide and only 36% in the US.

    According to Koomey, the reduced growth rates over earlier estimates were, driven mainly by a lower server installed base than was earlier predicted rather than the efficiency improvements anticipated in the report to Congress. In the NY Times article, Koomey goes on say, Mostly because of the recession, but also because of a few changes in the way these facilities are designed and operated, data center electricity consumption is clearly much lower than what was expected

    However, this reduction in growth is likely temporary, as our appetite continues to increase for internet access, streaming and cloud based services. Data centers will continue to consume growing amounts of electricity, more and more data centers will come on line, and data center managers will increasingly look to newer technologies to reduce their ever growing electricity bills. Additionally, when you consider that the estimated energy consumption of the US in 2010 was around 3,889 Billion kWh, 2% still represents close to 78 billion kWh. Clearly the trend is increased data consumption and with it increased energy consumption.

    In light of these trends and despite the lower growth rates, many industry insiders are continuing to turn a critical eye toward cooling, recognizing both the inefficiencies of current approaches and the improvements possible through new technologies. The information contained herein is designed to assist the data center professional who, while keeping uptime and redundancy inviolate, must also balance growing demand for computing power with pressure to reduce energy consumption.

    Issue: Understanding the Efficiency Metrics Best Practice: Adoption and use of PUE/DCiE

    In furtherance of its mission, The Green Grid is focused on the following: defining meaningful, user-centric models and metrics; developing standards, measurement methods, processes and new technologies to improve data center performance against the defined metrics.

    Measurements like watts per square foot, kilowatts per rack, and cubic feet per minute (CFM) are ingrained in data center dialogue. Until recently, no standard measurement existed for data center efficiency. Enter the Green Grid, a consortium promoting responsible energy use within critical facilities. The group has successfully introduced two new terms to the data center lexicon: Power Usage Effectiveness (PUE) and Data Center Infrastructure Efficiency (DCiE).

    Power Usage Effectiveness (PUE)

    PUE is derived by dividing the total incoming power by the IT equipment load. The total incoming power includes, in addition to the IT load, the data center s electrical and mechanical support systems such as chillers, air conditioners, fans, and power delivery equipment. Lower results are better, as they indicate more incoming power is consumed by IT equipment instead of the intermediary, support equipment.

    While it s not the only consideration, cooling can be a major player in PUE measurement. Consider the following diagram, where the combination of the chiller, humidifier, and CRAC consume 45% of the total energy coming into the facility.

    The Uptime Institute approximates an industry average PUE of 2.5. Though there are no tiers or rankings associated with the values, PUE allows facilities to benchmark, measure, and improve their efficiency over time. Companies with large-scale data center operations, like Google and Microsoft, have published their PUE. In 2008, Google had an average PUE of 1.21 across their six company data centers. Microsoft s new Chicago facility, packed with data center containers, calculated an average annual PUE of 1.22.

    The widespread adoption of PUE, left in the hands of marketing departments, leaves the door open for manipulation. Though the equation seems simple, there are many variables to consider, and users should always consider the context of these broadcasted measurements. At its core, however, the measurement encourages the benchmarking and improvement at the site level-the actions individual professionals can take to improve the efficiency of their facilities.

    Data Center Infrastructure Efficiency (DCiE)

    DCiE is simply the inverse of PUE-Total IT Power/Total Facility Power x 100%. DCiE presents a quick snapshot into the amount of energy consumed by the IT equipment. To examine the relationship between PUE and DCiE, A DCiE value of 33% (equivalent to a PUE of 3.0) suggests that the IT equipment consumes 33% of the power in the data center.

    Approximately 44 million Americans owe student loan debt, totaling $1.4 trillion collectively. How much individual students will end up owing differs widely, however. The state and individual school a student attends can make a massive difference, according to a new in-depth analysis of the student loan situation in the U.S. from LendEDU, an online marketplace for student loans and student loan refinance.

    Using financial aid data from Peterson’s, it has compiled an extensive report that details which states have the highest and lowest average student loan debt and which four-year schools are the most likely to leave grads with high or low levels of debt. All data included was self-reported, and was not verified by Peterson’s or LendEDU. Still, the data does provide important insights into student loan debt.

    The survey is based on students who both started at their college as first-time students and graduated from it with a bachelor’s degree. Here’s a look at the study’s most significant findings. This was LendEDU’s second annual report on student debt, so we can compare these statistics to last year’s, as well. (For more, see ​All About Student Loans.)

    Student Loan Debt: The Big Picture

    To put things in perspective, LendEDU’s research team looked some of the broader trends affecting borrowers. Specifically, it pointed out the following:

    • 59.78% graduates had student debt in 2016. That’s among students at both four-year nonprofit public colleges and private colleges and universities.
    • The average borrower in the class of 2016 had student loan debt of $27,975. The average grad overall had $17,112 in student debt.
    • Students at private schools had an average of $30,281 in loan debt, while students at public colleges averaged $26,828 in loans.
    • Among the 250 schools where students had the highest amount of loan debt, 80% were private institutions.
    • At schools where grads had the least amount of loan debt, 47% were public.

    ​So how does that compare to the class of 2015? According to LendEDU’s report from last year, 59.63% of seniors who graduated from public and nonprofit colleges in 2015 had student loan debt. So, the proportion of students graduating with debt has increased slightly. The average debt of students decreased 1.5% percent, though, from $28,400. This was a decrease from 2014, when The Institute for College Access and Success (TICAS), which also uses data from Peterson’s, put the average student debt load at $28,950 per borrower in 2014. Over the last two years, the average debt per borrower has dropped almost $1,000. However, the average graduate debt increased 6.73% from $16,033 to $17,112.

    With statistics like these, it’s sort of a mixed bag. LendEdu CEO, Nate Matherson was optimistic, though: “Over the last year I think we’ve seen a greater hesitation of institutions to raise tuition, and colleges putting more thought into the rate of tuition increases. And also, I think families are borrowing more responsibly, and a big piece of this is education.” Matherson was quick to note that student debt was “still a huge problem,” but, for these reasons, there might be some hope when it comes the future of student debt.

    One surprising finding worth pointing out is the gap between borrowers at private schools and those attending public colleges and universities. The College Board puts the average annual cost of tuition and fees at four-year private universities at $33,480. By comparison, average annual tuition at four-year public universities for out-of-state students comes to $24,930.

    With such a big difference in cost, it seems logical to expect that borrowers at private schools would be taking on substantially more debt. The fact that there’s less than a $5,000 difference between private and public school borrowers could mean that private universities are offering better scholarship or need-based grant options, leaving students needing to borrow less money.

    Student Loans by State: Who Has the Most/Least Debt?

    Beyond those details, the LendEDU report also delves into which states have the biggest and smallest average student loan debt totals. At the high end you’ve got Pennsylvania, where the average borrower owed $35,185 in 2015. At the low end is Utah, where the average student loan debt came to $18,810. Utah also had the lowest percentage of borrowers owing student loan debt, at 39%. Generally speaking, students in eastern states experienced the highest debt loan and those in the west, the lowest.

    However, it’s West Virginia that has the highest proportion of borrowers with loan debt. Overall, 76% of students attending West Virginia schools owed student loans. New Hampshire ad South Dakota came in a close second, at 75%. That may be because these students have fewer financial resources and are more likely to borrow.     With dozens of health care facilities in nearly every state, Universal Health Services (UHS) isn t quite ubiquitous, but it s working on it. One of the largest for-profit hospital operators in the nation, UHS owns or leases about 25 acute care hospitals with a total of 5,500 beds, primarily in midsized rural and suburban communities. The company also more. operates several outpatient surgery centers and radiation treatment facilities, most of which are located near its acute care hospitals. In addition, UHS behavioral health division operates about 200 psychiatric and substance abuse hospitals with a combined capacity of about 20,000 beds. The company is controlled by founder and CEO Alan Miller.

    UHS acute care facilities are located in more than half a dozen states and are situated mostly in smaller towns and cities with limited competition, though the division does have facilities in a few larger markets (such as Las Vegas and Washington, DC). UHS behavioral health hospitals are scattered across about 40 US states, as well as Puerto Rico and the US Virgin Islands. The company s biggest markets for both segments are California, Nevada, and Texas, which together account for nearly half of the company s revenue.

    UHS receives about 55% of its annual revenues from its acute care segment, which includes medical hospitals, surgical outpatient facilities, and radiation oncology centers. The remainder of the company s revenue comes from its portfolio of behavioral health hospitals, which includes residential facilities for teens, adult psychiatric hospitals, substance abuse facilities, and special education schools for students with emotional problems.

    The company s behavioral health business accounted for a larger portion of sales in 2011 (45%, up from 30% the previous year) due to UHS acquisition of Psychiatric Solutions in late 2010.

    Sales Marketing

    Both of UHS operating segments (acute care hospitals and behavioral health hospitals) earn between 40% and 50% of revenues from managed care providers (HMOs, PPOs, Medicare Advantage, and other commercial plans), with the remainder of sales coming from traditional Medicare and Medicaid plans, self-pay customers, and other sources.

    UHS growth strategies have helped it to steadily increase sales over the past decade. The firm saw its revenues jump by 35% to some $7.5 billion in 2011, largely due to the Psychiatric Services acquisition the previous year. Net income levels also rose by more than 70% to $398 million that year, reaffirming the company s profitability after a slight dip in net income levels in 2010 (largely due to acquisition costs).

    While the company s growth strategy is to build or purchase new facilities in rapidly growing areas — it has grown both of its units through selective acquisitions and construction efforts over the years — UHS also has no qualms about ridding itself of operations that just don t quite fit anymore. The company sold some $118 million and $21 million in assets in 2011 and 2010, respectively; divestitures in 2011 included several behavioral health facilities that the FTC required UHS to sell following the Psychiatric Solutions acquisition. In 2012 the company exited the Washington State acute care market through the sale of its Auburn Regional Medical Center.

    By focusing its operations on high-growth regions, UHS also works towards its goal of increasing hospital utilization rates (which is often a key indicator of the financial health of a hospital). To further draw more patients and high-quality physicians to its existing facilities, the company invests in new technology, makes capital improvements, and increases the breadth of services it offers. Initiatives include upgrades to surgical equipment and billing systems, as well as the implementation of electronic health record (EHR) systems to improve patient care coordination. UHS is especially expanding its outpatient service capabilities, as payers put pressure on hospitals to control inpatient care costs.

    Mergers Acquisitions

    The $3.1 billion acquisition of Psychiatric Solutions in 2010 doubled the behavioral health division s operations by adding roughly 100 mental health care facilities in more than 30 states. UHS had already been growing the division through smaller purchases.

    UHS further expanded the behavioral health business in 2012 by acquiring private psychiatric hospital provider Ascend Health for $500 million. The purchase added Ascend s nine freestanding psychiatric inpatient facilities in five states in the Southwest.

    Laws of Utah Drug Abuse

    Penalties for the possession of drugs in Utah vary according to the type of substance involved. Utah classifies its drugs into five schedules based on the likelihood of addiction and perceived dangerousness. Schedule I substances are considered most addictive, while schedule V drugs have the lowest risk of addiction.

    While drug penalties in Utah are tough, state officials are advocating treatment-based approaches in lieu of jail time. In October 2015, Utah passed a law to decriminalize first-time and second-time schedule I and schedule II drug possession offenses. Charges for these offenses were reduced from third-degree felonies to class A misdemeanors. The law also decreases the size of drug-free zones where violators would normally receive increased charges. With these changes to the previous laws, Utah hopes to see a decrease in repeat drug-related offenses and an increase in people getting treatment.

    Load density, air distribution, floor tile positioning; data center design is more complicated than ever, but with some best practice considerations, creating an efficient, reliable data center design is within your grasp.

    Let’s explore 10 Important Data Center Best Practices:

    1. When designing your data center, consider initial and future loads; in particular part and low-load conditions .
  • Lower data center power consumption and increase cooling efficiency by grouping together equipment with similar heat load densities and temperature requirements. This allows cooling systems to be controlled to the least energy-intensive set points for each location.
  • As a long-time data center professional, I encourage all my customers to reference the “2011 ASHRAE Thermal Guidelines for Data Processing Environments” to review the standardized operating envelope for the recommended IT operating temperature.
    • Identify the class of your data center to determine the recommended and allowable environment envelopes:
    • “Recommended” combines energy-efficient operation with high reliability
    • “Allowable” outlines boundaries tested by IT equipment manufacturers for functionality.
    • Keep in mind that operating outside the recommended envelope may cause server fans to operate at higher speeds and therefore consume more power .
    • Higher return air temperatures extend the operating hours of air economizers
    • Higher return air temperature improve cooling infrastructure efficiency, saving both energy and money.
  • Implement effective air management to minimize or eliminate mixing air between the cold and hot air sections. This includes configuration of equipment’s air intake and heat exhaust paths, location of air supply and air return and the overall airflow patterns of the room. Remember to create barriers and seal openings to eliminate air re-circulation. Supply cold air exclusively to cold aisles and pull hot return air only from hot aisles.
  • Under-floor and over-head cable management is important to minimize obstructions within the cooling air pattern.
  • Using fan speed control to supply only as much air as IT equipment requires can reduce energy use by up to 66%
  • Carefully consider the location of floor tiles to optimize air distribution and prevent short-circuiting.
  • Managing a uniform static pressure in the raised floor, by careful placement of the A/C equipment, allows for even air distribution to the IT equipment.
  • Create a low pressure drop design to minimize fan power consumption by keeping ducts as large and short as possible, in addition to a generous raised floor.
  • Incorporate some form of economizer whenever possible to save money, energy, and the environment.
  • By utilizing these data center best practices, I believe you are well on your way to a successful design!
